Lynda is certified to administer the Canadian Physical Activity, Fitness and Lifestyle Approach (CPAFLA) personal fitness appraisal. This fitness appraisal assesses body composition, aerobic fitness, strength, flexibility, back health and lifestyle habits. Lynda uses the results obtained to determine the optimal course of action for your current condition and to design a unique Personal Training Program to meet your future goals. A fitness appraisal is the first step in a comprehensive and personalized fitness plan that Lynda will guide you through.

- Physical Activity Readiness Questionnaire (PAR-Q)*
- Adult Consent Form
* If you answer YES to one or more of the questions on the PAR-Q, before you have a Certified Fitness Appraisal, you must obtain a note from your doctor approving your plans. The PAR-Q is valid for a maximum of 12 months from the date it is signed and becomes invalid if your conditions change so that you would answer YES to any of the seven questions.
